I affirm that I am at least 21 years of age and have agreed to accept a minimum of at least the next two wine club shipments. I further acknowledge that this wine club membership will continue from year to year until further notice, and I hereby authorize David Coffaro Winery to automatically charge my credit card account each time.

I also agree to keep David Coffaro Winery updated with any change in the information I have provided, including an accurate and up to date email address where I can be notified in advance of each wine club shipment. I acknowledge that wine club shipments will automatically be sent twice a year, once in April and once in October, unless I have elected to pick up my allotment at the winery. 

Cancellations may be requested at any time after the first two shipments have been received, and a notification of cancellation must be sent in writing to pat@coffaro.com. Cancellation requests not acknowledged within three days of notification, are to be considered unconfirmed.
